Optimizing Medication Processes Through Interdisciplinary Mental Health Collaboration

This session uses a real-world case to examine how collaboration between a community pharmacy at a mental health center, and a primary care clinic can improve medication safety and transitions of care in outpatient mental health settings. Attendees will learn how interdisciplinary teams can identify medication-related risks, address communication gaps, redesign medication-use systems, and overcome shared barriers to better support patients with complex mental health needs. This discussion highlights system-level strategies that improve transitions and continuity of care, reduce medication-related risks, and strengthen care coordination.
