Experts from Avant Pharmacy & Wellness Center walk through the art of successful implementation of non-face-to-face services such as chronic care management, remote physiologic monitoring, behavioral health integration, and principal care management.
Following the training will be an on-site tour of Avant Pharmacy & Wellness Center where members of the Avant Pharmacy team, collaborating medical providers, and program instructors will be available to answer your questions as they walk through the pharmacy setup – medical practice, pharmacy, wellness area, and more.

1. Describe how pharmacist-led clinical services impact Centers for Medicare and Medicaid Services initiatives for the quadruple aim and value-based payment models.
2. Explain the collaborative relationship between a pharmacist and billing provider for non-face-to-face services.
3. Describe the difference between complex and non-complex chronic care management (CCM).
4. Explain the eligibility criteria, billing, and documentation requirements for CCM.
5. Define RPM and qualifications for RPM as outlined by the Centers for Medicare & Medicaid Services (CMS).
6. List the required components of RPM treatment management services.
7. Describe the billing and documentation requirements for RPM.
8. Explain the eligibility criteria, documentation, and billing requirements for behavioral health integration (BHI).
9. Compare and contrast general BHI to Psychiatric Collaborative Care Services (CoCM).
