The Community Health Workers (CHWs) in Community-based Pharmacy Care Delivery educational modules are intended to help community-based pharmacists and staff members gain an understanding of how CHWs can be a health resource to individuals in their communities in addition to assisting them with their medication needs. This educational series is a 3-hour self-paced activity, in which each module discusses the roles of CHWs, including first-hand experiences from community-based pharmacists and CHWs.

Learning Objectives

  1. Describe the role of Community Health Workers in healthcare delivery systems.
  2. Identify the role of Community Health Workers specifically in community pharmacy care.
  3. Explain the principles of team-based care.
  4. Identify social determinants of health and their characteristics in different communities.
  5. Describe the impact health disparities have on overall health and well-being.
  6. Assess patients for social determinants of health.
  7. Identify appropriate points of hand off or referrals to support agencies.
  8. Describe the process of referral resource identification.
  9. Explain visit reporting and record keeping.
  10. Discuss method(s) and importance of referral follow-up.
  11. Identify the importance of establishing communication as a foundation to improve care.

  18. Summarize the process of home visitation consent and ethical requirements for the visitor.
  19. Discuss the considerations needed to ensure personal safety and establish boundaries.
  20. Demonstrate skills for a healthy life balance through understanding and managing empathy.
