Community Health Workers in Community-Based Pharmacy Care Delivery
The Community Health Workers (CHWs) in Community-based Pharmacy Care Delivery educational modules are intended to help community-based pharmacists and staff members gain an understanding of how CHWs can be a health resource to individuals in their communities in addition to assisting them with their medication needs. This educational series is a 3-hour self-paced activity, in which each module discusses the roles of CHWs, including first-hand experiences from community-based pharmacists and CHWs.

- Describe the role of Community Health Workers in healthcare delivery systems.
- Identify the role of Community Health Workers specifically in community pharmacy care.
- Explain the principles of team-based care.
- Identify social determinants of health and their characteristics in different communities.
- Describe the impact health disparities have on overall health and well-being.
- Assess patients for social determinants of health.
- Identify appropriate points of hand off or referrals to support agencies.

NCPA is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program will provide 3.0 contact hour (0.3 CEU) of continuing pharmacy education credit. The participant will need to complete the components listed above including a pre-test, video(s), final assessment, and an activity evaluation. Each final assessment is considered complete with a grade of 70% or better. NCPA’s continuing pharmacy education policy provides you with two opportunities to successfully complete this continuing pharmacy education assessment. Please note that you will not be permitted to submit the assessment a third time.
